Building on Tower Health’s foundation of high-quality, compassionate care, Tower Health at Home offers patients a seamless continuum of care following inpatient discharge, as well as ongoing management of chronic conditions, in their own home through a variety of home health services, including skilled nursing care, therapy, hospice, and home care.

Joining forces to strengthen community care

Tower Health at Home was created when Home Health Care Management, Inc. (HHCM), the parent company of Berks VNA, Pottstown VNA, and Advantage Home Care, became part of Tower Health on January 1, 2019. At the same time, the Berks branch of Affilia Home Health joined HHCM, and therefore also became part of Tower Health at Home.

Serving patients across southeastern Pennsylvania

Tower Health includes Reading Hospital in West Reading; Chestnut Hill Hospital in Philadelphia; Phoenixville Hospital in Phoenixville; Pottstown Hospital in Pottstown; and St. Christopher’s Hospital for Children, a partnership of Tower Health and Drexel University in Philadelphia. It also includes Reading Hospital Rehabilitation at Wyomissing; Reading Hospital School of Health Sciences in West Reading; home healthcare services provided by Tower Health at Home; and a network of 22 urgent care facilities across the Tower Health service area. Tower Health offers a connected network of 2,200 physicians, specialists, and providers across more than 230 convenient locations.
